You can download the Primary Nomination Form below. Each form is for a specific county, click on your county to download the right form for you.

When getting signatures be sure to that all of the signatures on one form are from the same county.

Dr. Pat Bertroche will need signatures from at least six counties. Each page of the petition may have signatures from only one county. The minimum number of signatures that must be collected in each of the six counties is equal to two percent (2%) of the votes cast for President by the voters of that county in the 2008 General Election, rounded up to the next whole number. The number of signatures that must be collected in each of the six counties in the Third Congressional District is listed below.
